Clean up the misc/formater.hh file.

* src/misc/formater.hh (formater::format, formater::prime): Move ...
* src/misc/formater.cc: ... in this new file.
* src/misc/Makefile.am: Add it.
* src/sanity/style.test: Allow <iostream> in headers where << or >> are
used.
